If your child requires medication/s such as an inhaler, epinephrine, etc., you will need to have your child's health care provider fill out and sign the Permit To Take Medication form. This form can be found here (or on the Moreland School District Website under "News and Resources" in the Health Section). Medication must be supplied by the parent in the original prescription/pharmacy container, clearly labeled with the student's name, medication, dosage, and directions, or original sealed over-the-counter container.
